- Using your own SMU credentials, login to the health center portal.
- Once logged in, click on “Dependents,” located on the left menu towards the bottom.
- A new screen will appear. Click on the “Add New Dependent” button.
- Fill the required fields and hit the “Submit” button.
- Once the new profile is created, the dependent’s name and birthdate will appear in the menu. Click on the “Launch” button to the right of the dependent’s information.
- When you click on “Launch,” the portal will take you to your dependent’s scheduling page. Select “Select or Schedule an Appointment” and follow the prompts.
