SMU ORGANIZATIONS AND DEPARTMENTS: BEFORE SUBMITTING THIS FORM, ENTER STABLE TO SUBMIT A RESERVATION REQUEST TO DETERMINE SPACE AVAILABILITY. 

Please complete the following form regarding your Reservation Request or Special Event inquiry. This information will be used to determine the ability and feasibility of your request.

SUBMITTING THIS FORM IS NOT AN APPROVED RESERVATION. CONFIRMATION OF RESERVATION WILL COME THROUGH STABLE.

Student Organizations using external to SMU vendors, contractors, volunteers, or instructors MUST provide a copy of the approved Contract Information Form (available at SMU Connect) to Campus Recreation prior to approval for event.

If you using an external-to-SMU vendor, contractor, instructor, or volunteer, you must submit a Contract Information Form to Student Activities via the SMU Connect webpage:

It is the organization's responsibility to provide proof of approval to SMU Campus Recreation Events and Reservations no later than 3-Business Days (Monday-Friday) to date.

Failure to provide proof of approved Contract Information Form will result in cancellation of request.

As the Requester and representative of the organization or department above, I understand that providing false or misleading information regarding our request for space in the Dedman Center will result in immediate termination of event request or event and additional sanctions against the organization/department and the Requester including but not limited to: disciplinary case, expiration of reservation privileges, Conduct proceedings, and possible criminal charges.
