Dear SMU employees,
Please upload your COVID-19 vaccine information by today through the secure University health center portal to provide proof of vaccination.
If you received your vaccine at the Dr. Bob Smith Health Center or already uploaded your card or other proof of vaccination, no further action is needed. Click here for steps to check if you are unsure.
If you have not yet received your vaccine, time is running out to meet the federal requirement deadline. To be “fully vaccinated” by January 18, 2022, you must receive the first dose by:
- December 7 for Moderna (two doses, four weeks apart)
- December 14 for Pfizer/BioNTech (two doses, three weeks apart)
- January 4 for Johnson & Johnson (single dose)
Or by December 17, you can request a religious or medical accommodation as an exemption.
Although not required, booster shots are also now available on campus for anyone age 18 or older.
